Is the $40 fee a one-time or reoccuring cost? Are there any other charges I need to be aware of?

The $40.00 fee covers an entire year of service and reoccurs annually should you choose to continue using the service. There are no other charges beyond this fee (no monthly charges, no per player charges).

I manage two teams. Do I have to pay for both teams, or does the fee cover both teams?

You would need to pay the $40 for each team that you register, so 2 teams @ $40 each = $80.

Is there a Windows Phone 7 app?

No. At this time, there are only apps for the Android OS and the iPhone, iPad, iPod Touch.
